Patient Navigation in Cancer Care Tool Kit
With the helpful guidance from a team of experts in the field of cancer care, Pfizer
Inc, in cooperation with the Healthcare Association of New York State (HANYS) and
Dr Harold P. Freeman, president and founder of The Ralph Lauren Center for Cancer
Care and Prevention, developed the Patient Navigation in Cancer Care: Guiding patients
to quality outcomes™ multimedia tool kit. The tool kit includes training
manuals to provide resources and support to program champions—individuals who initiate
and support implementation of a patient navigation program within a given organization—and
to prepare patient navigators for their responsibilities.
This resource contains 3 manuals in an attractive housing unit:
- Establishing a Patient Navigation Program: An Implementation Guide for the Program
Champion
Designed to assist the champion in promoting the concept of navigation within an
organization and provides resources to help implement the program
- Navigation Pathways: The Patient Navigator Training Manual
Clearly defines the role and responsibilities of patient navigators and provides
resources to help them guide patients through the health care system
- Breast Cancer Pathways: A Resource Guide for the Patient Navigator
Contains appropriate information specific to breast cancer to educate navigators
on the special considerations of this disease and provides the tools and resources
necessary to assist patients with breast cancer
