Freeman, president and founder of The Ralph Lauren Center for Cancer Care and Prevention, developed the Patient Navigation in Cancer Care: Guiding patients to quality outcomes™ multimedia tool kit. The tool kit includes training manuals to provide resources and support to program champions—individuals who initiate and support implementation of a patient navigation program within a given organization—and to prepare patient navigators for their responsibilities.

This resource contains 3 manuals in an attractive housing unit:

• Establishing a Patient Navigation Program: An Implementation Guide for the Program Champion
  Designed to assist the champion in promoting the concept of navigation within an organization and provides resources to help implement the program
• Navigation Pathways: The Patient Navigator Training Manual
  Clearly defines the role and responsibilities of patient navigators and provides resources to help them guide patients through the health care system
• Breast Cancer Pathways: A Resource Guide for the Patient Navigator
